Custom Pets Dog Training Class Registration

To register for a class, please print out the registration form and fill in the following information. Indicate the class desired, and sign and date the form. Mail the completed form with your payment to:

Once we receive your completed registration form and payment for class, we will send you a confirmation which will include what to bring to class and driving directions. If you have any questions, or wish to verify that there is an available slot in the class you choose, please contact Lori at lori@custompets.com.

I understand that attendance of a dog obedience class is not without risk to myself, my dog, or to members of my family. I certify that my dog is in good physical condition and friendly towards dogs and humans, and agree to hold Custom Pets Dog Training, its employees, and its agents harmless of any injuries to myself, my dog, members of my family, or guests who accompany me to class. I further agree to accept full responsibility if my dog causes injury to a person or another dog.
Refund Policy: I understand that I may request a full refund up to 72 hours prior to the class start date/time. If I attend the first session, I may request a full refund during or at the conclusion of that session. I understand that no refunds will be given after the first class session.
